We are so excited that you have chosen us for your drop-in childcare needs and look forward to your first visit! If you have any issues with registering online, please call us at 513-661-PLAY.
Follow the steps below to begin!

To begin our registration process, you will need to fill out the following forms and bring with you to Play4Hours (click the links below):

Child Enrollment & Helath Information (needed prior to child's first day)
​
Medical Statement (needed within 30 days of child's first day)
​
Care Plan for Children with Special Needs
**this form only needs filled out if your child needs special accommodations; at this time we do not administer any type of medications**

Policy & Procedure Handbook
**Please review this copy of our Policies and Procedures. The signature needed that identifies you have reviewed this handbook is located on the last page of the Child Enrollment and Health Information. We have extra copies of our Policy and Procedure Handbook at Play4Hours if needed.**
*You will be redirected to the Procare Solutions online registration form for Play4Hours. This is the app we use to communicate with the caretakers while the child is in our care.

**There is a $25/child Registration Fee ** Families with vouchers do not pay a Registration Fee. They can Register by coming into the daycare. ​
​
*At least one authorized pickup must be added to each registration.*
